Not all of our physicians accept workers' compensation.  Advise the office staff prior to making your appointment.  We will assist you in making an appointment with a physician who accepts workers compensation. 

When you call to make an appointment, please be certain to have available the name of your employer, the date of injury, the claim number and the insurance adjuster's name and phone number if you have it. 

We will verify the workers' compensation claim and get an appproval for your visit, prior to scheduling an appointment.
